Lessons Learned From Fatality Assessment and Control Evaluations (FACE) Investigations in Michigan (AIHce EXP 2021 OnDemand)
Course Description:

This education session will present two case studies of confined space fatalities investigated by the Michigan Fatality Assessment and Control Evaluation (MIFACE) program. The first case involves the death of a worker in a trench collapse. The second case is that of a welder who died in an explosion while working inside of a cargo tank that had previously held fuel. Each case will discuss: a) background information about the workers and employers; b) the findings of the MIFACE investigation; c) the key factors contributing to the fatality; and d) recommendations developed to address these factors.

Upon completion of the session, the participant will be able to:
  • Identify types of confined space hazards, including trenches.
  • Recognize employer, employee, or work culture characteristics that can worsen confined space hazards.
  • Develop a plan for implementing controls to mitigate confined space risks.
