This presentation will discuss how too many times tragic confined space accidents can be attributed to a failure to promptly and adequately identify the adverse consequences associated with changes in confined space hazards and risks during a confined space activity. It is imperative that a confined space entry procedure and permit process be inclusive of a management of change (MOC) process that can identify changing conditions, hazards, and risks and provide for additional or changed hazard control measures. The MOC process contained within the new NFPA 350 Confined Spaces Guideline will demonstrate how a MOC process could have prevented three tragic confined space incidents. Presenters will show how to utilize regulations and consensus standards to ensure safe confined space entry.
